The year has flown by and I know that so many of our chapters, divisions and caucuses are active and have been doing some very exciting programming and networking. Now is the time to capture a picture of that activity by completing your annual report. For chapters and divisions, this is one of two reports which are required; the other will be the annual financial report. For caucuses, an annual report is the only report required. It only takes a few minutes of your time.
We’ve tried to make it very simple this year to complete and for you to check that it has been submitted correctly. This report is important on many fronts. First, it provides a synopsis of your activities and should be reported to your Board, members and archived. Second, it provides a snapshot to our cabinet chairs to review the overall health of our units and who may need some additional assistance. Third, your ideas may spark ideas for others; why reinvent the wheel? And finally, this report is required to ensure compliance with the Internal Revenue Service and other regulatory bodies. The deadline for submission is 1 December 2014. The Annual Report is to be delivered to SLA Headquarters, Leadership Central via the SurveyMonkey links below.
For reference purposes only, all questions contained within the survey can be found under each link as “Blank Survey.” This may be helpful in seeing the data required before beginning the actual survey. Please do not email or fax the “Reference Only” documents to headquarters.
How important is this report? It is mandated by the Bylaws and per the Reporting Requirement for SLA Units policy, that if any chapter, division or caucus fails to submit the Annual Report within two months of the due date they will forfeit their annual allotment and may be subject to dissolution.

Print for Your Records
Follow the steps below to print your unit report.
1. Click your unit link below (caucus, chapter or division). You’ll then come to a “Summary” page for all who have submitted a report. This may be helpful for benchmarking purposes.
2. Click the “Browse Responses” button in the upper right-hand corner to see individual responses.
3. In the upper right-hand corner, just below the “View Summary” button is “Displaying X of X respondents ( « Prev Next » ). Scroll through until you find your unit, and print.
